About Summit ElderCare:

Fallon Health operates the largest Program of All-Inclusive Care for the Elderly (PACE) in New England and the fifth largest in the country. Called Summit ElderCare, Fallon’s PACE helps provide older adults and their caregivers with a welcome alternative to nursing home care. Participants in Summit ElderCare have access to comprehensive medical services and social support at a Summit ElderCare site while they keep the independence of living in their own homes and communities.


Brief Summary of Position:

The primary responsibility of the Outreach Support Coordinator is to maximize productivity of the Sales team by providing sales and marketing support to the Outreach Account executives that assists in the growth and retention of membership. Additional responsibilities include targeting key sales opportunities through lead generation for the outreach team. Also this position will partner with the Sales Outreach team to support external community events.



  • Work closely with the Outreach Executives in creating the sales event calender including the registration, payment , delivery, set up and breakdown of event.      
  • Partner with the sales outreach team on external sales opportunities by attending community event and sales activities. (I.e. Health Fairs, Sponsorship events and Information Tables.) as needed
  • Build relationships with appropriate partners at local Senior Centers, Housing authorities and Nursing Homes. Drop by regularly to ensure all marketing materials are available and promote referrals and lead generation.
  • Provide daily reports on open sales tasks and help identify key leads for the outreach team and Management.
  • Track and report on sales activities; ensure accuracy of entries.
  • Document and update leads into the Sharepoint CRM. Report regularly on open leads.
  • Update and maintain the calenders for all outreach staff. Provide updates to the outreach team when new opportunities arise...
  • Establish strong relationships with internal departments within Fallon Health
  • Handle in bound calls from our external customers in support of education and sales.
  • Work with management and the outreach team on identifying new business partners that show an interest in referring to Summit ElderCare
  • Review all enrollments to ensure complete information has been provided.
  • Adhere to regulatory guidlelines re: Medicare, HIPPA, and Fallon Health patient confidentiality.
  • Maintains a personal touch in all correspondance and communication with prospects.    
  • Work very closely with team members to solve customer problems. Offers solutions and suggestions for process and product improvement to management. Determine, understand and meet prospect needs.
  • Assist in training activities as needed.
  • Comply with Corporate, CMS and department policies and procedures.
  • Support the Account Executives with any administrative requests that helps support our Membership goals.
  • Responsible for the preperation/compilation of reports related to the departments key objectives.

  • Become an expert on Sharepoint CRM.

  • Create ROI reports on all marketing campaigns for Summit ElderCare.

  • Assist in retrieval of MA Health/Financial documents from prospects

  • Create Inventory and needs list for marketing collateral and supplies for Outreach Team

  • Stay abreast of trends within industry and share with management



Bachelor’s degree or equivalent combination of education and experience




  • 3 years’ experience in sales or service environment
  • Previous experience coordinating sales activities and events as well as providing support in the field.
  • Strong knowledge of Microsoft Office products
  • Knowledge of Customer Relationship Management (CRM) tool(s)
  • Demonstrate strong customer service skills and willingness to “own” a prospect from start to finish
