Fallon Health’s – Summit Elder Care program one of the largest PACE programs (Program of All-Inclusive Care for the Elderly) in the country and the very first PACE program in the nation to be associated with a Health Plan. Summit ElderCare currently already serves residents of Hampden County, Worcester County, and the communities of Easthampton, Granby, Hudson, Marlborough, Southampton and South Hadley. We help give elderly adults and their caregivers an innovative choice in health care as a welcome alternative to nursing home care. Participants in Summit ElderCare have access to most medical services through a Summit ElderCare site while they keep living in their own homes and communities.

Provides personal care at the Center and in home settings to participants. Assists in all phases of the daily operation of the PACE program to enhance the ability of our participants to continue living safely in the community.
Sunday through Thursday 6:30am – 3pm (Sunday home care cases entire shift; Monday through Thursday 6:30am until 8:30am and then to the SE site until 3pm)
Sunday through Thursday 10am – 6:30pm (Sunday home care cases entire shift; Monday through Thursday 10am until 4:30pm at the SE site; 4:30 until 6:30pm home care)
Tuesday through Saturday 6:30am – 3pm (Tuesday through Friday 6:30am until 8:30am and then to the SE site until 3pm; Saturday home care cases entire shift)
Tuesday through Saturday 10am – 6:30pm (Tuesday through Friday 10am until 4:30pm at the SE site; 4:30 until 6:30pm home care; Saturday home care cases entire shift)
Education: High school graduate or equivalent.
Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program (training &
certification must be completed/obtained prior to employment).
License/Vehicle: Valid driver’s license with reliable transportation
Certification: Certification as a Home Health Aide or Nursing Assistant; CPR and First Aid Certification or willingness to be certified with 60 days of employment.
Experience: One year experience working with frail elders in a health care setting and one year of home care experience.
