About Fallon Health Weinberg - PACE:

Fallon Health Weinberg-PACE is a long-term care program for older adults who need a nursing-home level of care, but wish to stay living at home and in their community. We are a Program of All-inclusive Care for the Elderly (PACE), providing complete medical care and support services so you or your loved one can remain living safely at home

All care is coordinated and provided by an interdisciplinary team specializing in geriatric care, who work with participants and their caregivers to create an individualized care plan.

 

Brief Summary of purpose:

Positively impact FHW-PACE referrals and enrollments/census through outreach, education, and marketing efforts.

Responsibilities

  • Manage Outreach activities specific to FHW-PACE, including referral sources, potential enrollees and family members. Schedule appointments and group presentations.
  • To coordinate evaluations for potential participants in order to promote the best potential for a positive experience for participants:
    • Offer and conduct tours of the FHW-PACE Center to prospective participants, families, referral sources, etc.
    • Work closely with Center Manager to coordinate and facilitate the Intake & Enrollment process.
    • Work with Service/Transportation Coordinator to schedule transportation for potential participant visits to the Center.
    • Provide written and verbal information regarding all potential participants scheduled for intake on a timely basis to the Interdisciplinary Team.
    • When possible and/or appropriate, provide and obtain necessary information to families to ensure helpful facilitation of the intake process:
      • Financial statement
      • Medical release
      • MA application
      • Any other necessary information
      • Conflict Free Assessment
    • Coordinate and communicate necessary information with PACE Director on ongoing basis regarding potential enrollments
    • Coordinate with necessary agencies or facilities to insure appropriate communication and obtain pertinent information
    • Provide Weinberg Campus housing contact information as necessary
    • Maintains statistics on enrollment conversions and update the team weekly
  • Maintain vital communication and documentation:
    • Direct liaison with the DSS to coordinate and provide necessary information regarding potential enrollees
    • Coordinate with necessary agencies and/or facilities to ensure appropriate communication with regard to intakes/visits and obtain pertinent information when applicable.
    • Provide ongoing communication with referral sources and/or families:
      • Updates during intake process
      • Provide written/verbal follow up to referral sources regarding outcome of intake process or referral
    • Provide necessary documentation on Intake & Assessment form or referral for any participants that were not accepted into FHW PACE plan
    • Update initial information in to EMR (NextGen)
    • Verify all insurance information and that Medicaid is active and appropriate for FHW PACE (ePaces)
    • Assist in amendments to the FHW PACE required Marketing Plan
    • Participate and coordinate information for the CMS Audit.
    • Provide information for the quarterly MLTC report in the area of enrollment and rebalancing efforts
  • Identify new enrollment opportunities for FHW PACE
  • Meet monthly enrollment budget
  • May perform other duties as assigned by PACE Program Director

Qualifications

Education:

Bachelor’s degree or advanced education in health care strongly preferred

 

License/Certifications

N/A

 

Experience:

  • Two years professional work experience with sales and closing deals to achieve enrollment targets.
  • Strong knowledge of managed long term care, nursing home, assisted living and hospital admissions and discharge processes and other long term care environments. Knowledge of Medicare and Medicaid
  • Familiarity with state UAS-NY and NYS Enrollment Broker procedures
  • Familiarity with managed care concept
  • Excellent verbal and written communication skills
