Position Overview:
Our Physicians have flexibility to devote as much time as needed for the clinical and social needs of our participants and their caregivers to provide individualized quality care by a team of geriatric care professionals who work together with participants and caregivers to address each individual's specific needs. This team of professionals is an essential component of the Summit ElderCare PACE program.
The team uses a collaborative approach to care planning and is called the Interdisciplinary Team (IDT). The IDT is comprised of Providers (MD/DO and NP/PA), Nurses, Social Workers, Physical and Occupational Therapists, Dieticians, Health Aides, Recreation staff, Home Care and Transportation Coordinators and Site Administrator. This approach to care is the gold standard for complex and frail older adults who wish to avoid nursing home placement.

Brief Summary of Purpose:
The Care Coordinator or Navigator partners with Fallon Health Care Team staff and other providers to communicate at all times what is occurring with the member and their status. The Care Coordinator or Navigator seeks to establish telephonic and face to face (depending upon product and circumstance) relationships with the member/caregiver(s) and provider partners to better ensure ongoing service provision and care coordination, consistent with the member specific care plan. In order to effectively advocate for member needs, the Care Coordinator or Navigator may make in home or facility visits (depending upon the product and circumstances) with or without other Care Team members to fully understand a member’s care needs.

Brief Summary of Purpose:
Principle responsibility of the Medicare 5 star Program Manager is to collaborate with Fallon Health’s 5 Star owner to produce dashboards, trend and surveillance reports for senior management and department leaders and teams, tracking individual metric measure and executed intervention performance. This includes, a thorough performance review of end-to-end process across all Stars measures, inclusive of KPI identification, accountability tracking and review as well as oversight of all LOB activities and plans. Additionally this role will work with business areas to design, develop, modify and test reports to support business needs and transaction management in support of Fallon’s 5 star rating goals. This includes engaging leaders and business owners to help define, shape and drive Stars performance. The analyst will leverage their analytic skill set and experience with large health care data sets to identify and communicate trends as well as prioritize and recommend improvements through data-driven analysis. The Medicare 5 star Program Manager will also be responsible for investigating specific areas of concern with 5 star performance. The objective of the Medicare 5 star Program Manager activities is to qualify Fallon Health performance with real time date with the goal of creating actionable activities to improve performance. It is also expected that this role proactively identifies and addresses scalability and performance bottlenecks to support efficiencies. They are consultative to the business and accountable for the quality and effectiveness of the data in the subject area across all related systems (QNXT, data warehouse) and are expected to be subject matter experts for this data.

The SE Secretary/Receptionist has overall responsibility for all daily front office functions. Ability to direct all calls to the appropriate department and staff and obtain and convey messages. Ability to handle a multi-line phone system efficiently with comparison and calmness. Responsible for the maintenance of records of transportation for the members of the program and maintain a good relationship with our transportation vendor. Serves as the primary contact for all transportation concerns. Responsible for all daily incoming and outgoing mail, faxes and copying needed for all departments.

Brief summary of purpose:
Provides program level guidance and recommendations relevant to the provision of clinical and nursing services. Oversees implementation of discipline specific policies and procedures. Responsible for the operations of the PACE center clinic. Oversees all nursing services provided to the participants at the center. The UM RN (Utlilization Management RN) will be working flexible remote - mostly from home but may be required for some in office meetings in Worcester, MA. The schedule is M - F 8 am to 5 pm.

Brief summary of purpose:
The UM Nurse uses a multidisciplinary approach to organize, coordinate, monitor, evaluate and modify plans of care and/or service requests, focusing on selected complex medical and psychosocial needs of FH members and their families. The UM Nurse is responsible for assuring the receipt of high quality, cost efficient medical outcomes for enrollees. This role works with Medical Directors, Authorization Coordinators, and Service Coordinators to perform pre-service, concurrent, and retrospective reviews for Acute Hospital and post-acute facility level of utilizing established Care Coordination polices and protocols, Fallon Health benefit criteria, applicable regulatory review criteria and nationally accepted criteria for medical necessity determinations.

The FHW Office Coordinator is responsible for all daily front office functions, including managing all incoming calls (transferring them to the appropriate department or taking messages), incoming mail, faxes, assisting with outgoing mail and faxes, serving as primary contact for all transportation concerns, maintenance of records of transportation for program participants and maintaining a good relationship with transportation vendor and supporting all departments in daily copying needs.

Position Overview:
Manages a primary care panel of participants and provides continuity primary health care to PACE participants that reside in the community or long-term care setting with a focus on goal oriented, person-centered care, disease progression, disease prevention, wellness and management of chronic and episodic illness consistent with the participant’s goals of care. Works in close collaboration with all members of the PACE Interdisciplinary Team.

Brief Summary of Purpose:
This position manages the distribution and tracking of Medicare Advantage and PACE regulatory and sub-regulatory guidance from the Centers for Medicare & Medicaid Services (CMS) via HPMS memos to ensure timeliness and applicable deadlines are met; this includes items such as the annual rate notices, plan reminders of required actions and deadlines, and manual revisions. Submits marketing materials to regulatory agencies as delegated. Support Medicare Programs by assisting or leading special projects such as CMS audit documentation collection and submission, reviewing Annual Notice of Change (ANOC) documents for accuracy and completeness, and this may include coordinating with departments throughout Fallon and with contracted vendors. Manages all aspects of developing and submitting all Medicare Advantage and Senior Care Options Evidence of Coverage (EOC), Schedule of Benefits (SBs) and Low-Income Subsidy (LIS) Rider documents. Leads the oversight of HPMS’ Complaints Tracking Module (CTM) for appropriate and timely responses with the CMS-defined timelines.

The FH authorization process is an essential function to FH’s compliance with CMS regulations, NCQA standards, other applicable regulatory requirements, and customer expectations. The FH Authorization Coordinator serves to administer the FH prior authorization process as outlined in the Plan Member Handbook/Evidence of Coverage, departmental policies and procedures, and regulatory standards. The Authorization Coordinator serves as a liaison between FH members and/or provider offices and FH with their authorization management issues. Thorough research, documentation, and corrective action planning must be established for each respective case and adjudication completed in accordance with existing regulations, policies, and standards.

Position Overview:
Our Physicians have flexibility to devote as much time as needed for the clinical and social needs of our participants and their caregivers to provide individualized quality care by a team of geriatric care professionals who work together with participants and caregivers to address each individual's specific needs. This team of professionals is an essential component of the Summit ElderCare PACE program.
The team uses a collaborative approach to care planning and is called the Interdisciplinary Team (IDT). The IDT is comprised of Providers (MD/DO and NP/PA), Nurses, Social Workers, Physical and Occupational Therapists, Dieticians, Health Aides, Recreation staff, Home Care and Transportation Coordinators and Site Administrator. This approach to care is the gold standard for complex and frail older adults who wish to avoid nursing home placement.
