About Summit ElderCare:
Fallon Health operates the largest Program of All-Inclusive Care for the Elderly (PACE) in New England and the fifth largest in the country. Called Summit ElderCare, Fallon’s PACE helps provide older adults and their caregivers with a welcome alternative to nursing home care. Participants in Summit ElderCare have access to comprehensive medical services and social support at a Summit ElderCare site while they keep the independence of living in their own homes and communities.
Responsible for the provision of occupational therapy services to Summitt ElderCare participants and oversees the services provided by the Certified Occupational Therapy Assistant. Performs assessments of participants’ physical and cognitive abilities and provides treatment as indicated. Participates in care planning and interdisciplinary team meetings. Provides all services in accordance with applicable regulations as well as Fallon Health and Summitt ElderCare policies and procedures.
Primary job responsibilities:
Education: Graduate of an accredited rehabilitation program in occupational therapy.
License: Licensed as an occupational therapist in Massachusetts
Certification: CPR certification or willingness to obtain certification within 60 days of hire.
Experience: At least one year experience working with a frail or elderly population in a health care setting. 2-3 years experience in home or outpatient settings preferred.
