Fallon Health Weinberg is a partnership between Fallon Health of Massachusetts and Weinberg Campus of Erie County, New York. Fallon Health Weinberg offers a Program of All Inclusive Care for the Elderly (PACE) to serve the health needs of dual-eligible residents of the Western New York counties of Erie and Niagara.

Under the direct supervision of the Program Director, the Clinical Nurse Manager, provides program level guidance and recommendations relevant to the provision of clinical and nursing services both at the PACE center and in the community as applicable. Oversees implementation of discipline specific policies and procedures in accordance with FH, FHW, NYS and CMS regulations. Responsible for the overall operations and workflows of the PACE center clinic in collaboration with the Weinberg clinic manager. Oversees all nursing services provided to PACE participants. Supervises and directs , Primary RNs, Home Care Coordinator RNs, , LPNs and Health Aides, dietician and supervisor of food service assigned to the PACE center; and directs and provides oversight to the PACE clinic manager and medical assistants within the clinic to ensure provision of patient care within PACE requirements. Functions as the “Manager in charge” in the absence of the Program Director as needed/assigned
salary $ 110K-116K yearly
Graduate of an accredited school of Nursing. BSN strongly preferred
License to practice as a Registered Nurse in the State of New York.
CPR certification or willingness to be certified.
