Brief summary of purpose: 

Perform a variety of medical records responsibilities. All appoint functions for our participants at the day center and prospective enrollees. In collaboration with clinic staff, establish own work plan and priorities, using and/or modifying established procedures to assure timely completion of assigned work in conformance with established departmental policies and standards.

Responsibilities

  • Establish new patient charts, maintain charts, and assist in closing records for participants that have either disenrolled or expired from the program.
  • File (scan/import) documents and faxes in charts expeditiously and accurately.
  • Request or fax medical records to/from various providers offices and assist with authorization to release medical information to and from providers.
  • Maintain an organized filing/scanning system of all patients’ medical records.
  • Initiate, implement, develop authorizations and referrals into the EHR system for all outside appointments and coordinate with transportation staff.
  • Booking of all outside consultation appointments and transportation for participants and send all documentation needed to providers.
  • Create transportation orders; call and fax all transportation changes to vendor(s)
  • Notify all needed departments of transportation changes, participant transport changes and cancellations.
  • Inform all participants and/or caregiver of all outside appointments that have been scheduled.
  • Inform/provide printed participants and/or caregiver of all special instructions that may be needed for outside appointments.
  • Scheduling and prepares charts for prospective enrollee to ensure completed in a timely manner in the clinic.
  • Retrieves and tracks all consults from medical appointments; maintains consults and closes out referrals in the EHR.
  • Assists clinic staff as office coordinator as needed.

Qualifications

 

Education

High school graduate or equivalent

 

License/Certifications

CPR certification or obtain certification within 6 months of hire.

Reliable transportation.

 

Experience

Preferred:

  • 2 years in customer service, secretarial, or receptionist position
  • Excellent written and verbal communication skill.
  • Ability to establish effective relationships with elderly patients and their caregivers.
  • Ability to work independently and exercise sound judgement.
  • Ability to establish working relationships with physicians and medical staff.

 Required:

  • Intermediate computer skills (Word, Excel, Power Point)
